Student Information Form
Though most communications is provided through the Director during instructional or rehearsal time, when needed this form provides us the information on how to contact students and parents to advise of unexpected changes to schedules, updates to activities and special notices. All participants are asked to provide this completed form to ensure you do not miss any important updates.
Field Trip Permission Forms
All students are transported to events via District bus throughout the school year. A completed district mandated permission form is required of each participant. Students will not be able to board the bus without it on file with the Director. It also provides important emergency contact information in the event it is needed to treat your student.
